What is Tramadol?

Tramadol is a centrally-acting opioid analgesic that is used to treat the pain of various types and intensities.

Tramadol indications for use

Tramadol is commonly used as an analgesic and is used to treat persistent moderate to severe pain. The analgesic potency is comparable to that of morphine.

It is also suitable for the treatment of neuropathic pain, for pain relief for fibromyalgia, and for pain relief for toothaches and headaches.

In people with osteoarthritis, tramadol reduces the intra-articular concentration of Substance P and interleukin-6. This significantly reduces the pain caused by inflammation in the joint.

More than 90 percent of Tramadol is absorbed after oral administration and has an absolute bioavailability of about 70 percent, regardless of food intake. The drug undergoes little first-pass metabolism, and about 20 percent binds to serum proteins.

Tramadol penetrates the blood-brain barrier and the placenta and is excreted with breast milk. The elimination half-life is about 6 hours regardless of administration.

Tramadol is mainly excreted via the kidneys, therefore a slight prolongation of the elimination half-life in hepatic and renal impairment should be expected.

Pregnancy and breastfeeding

If tramadol is to be used during pregnancy, it should be limited to single doses. Prolonged use should be avoided because tramadol penetrates the placenta and withdrawal symptoms may occur after birth in newborns.

Tramadol should not be used during breastfeeding. However, there is usually no need to stop breastfeeding after a single dose of tramadol.

Driving ability

Because tramadol use may cause drowsiness and dizziness, the ability to drive and operate machinery is impaired under these conditions.

Tramadol interactions

The following interactions are possible when taking Tramadol:

  • Tramadol should not be used concomitantly with MAO inhibitors.
  • Concomitant use of central depressants
  • Concomitant use of carbamazepine (enzyme inducer) results in decreased serum concentrations of tramadol
  • In mixed agonist/antagonist combinations (e.g., buprenorphine, nalbuphine, pentazocine), the analgesic effect of pure agonist may be impaired.
  • Coumarin derivatives (e.g., warfarin) - increased INR and ecchymosis are possible.
